Since the purpose of this form is to bring a problem to the attention of the school library media specialist and the teacher, it is important for it to begin with a positive statement such as "We welcome student use of the public library." The notification form should list frequently occurring problems such as all materials on the subject are on loan and not available; not enough materials are available to satisfy the number of students; the particular topic is too new or too specialized for there to be sufficient available materials; the assignment as written is unclear; or there are too many restrictions on the subject. This form can be copied for the student to present to the teacher, and a copy can be sent to the school library media specialist.
